Fentanyl in Blood: Trends and Redistribution in Harris and Orange Counties

Since the opioid crisis was announced in 2017, the number of fentanyl-related deaths and the need to understand postmortem blood tests have grown. One crucial factor in interpreting these cases is postmortem redistribution, where drugs move within the body after death. Previous studies on fentanyl redistribution either had small sample sizes or were done before the opioid crisis, which might cause conflicting results.

This study looked at fentanyl blood ratios from 748 cases in Harris County, Texas, and Orange County, California, from January 2009 to June 2022. Because the data wasn't normally distributed, a Kruskal-Wallis test was used. The findings showed significant differences in central/peripheral ratios between the two counties, before and after the epidemic, and when fentanyl was used medically versus not. Researchers also checked factors like age, gender, and polydrug use, but no clear patterns emerged. The study suggests that the rise in ratios could be linked to increased illicit fentanyl use, but more research is needed to pinpoint the exact cause.
