Unraveling Bile Acid Mysteries in Pregnancy

ICP, or Intrahepatic Cholestasis of Pregnancy relies on TBA ( Total Bile Acid) levels to manage the situation. The decision on how and when to gauge TBA are unclear. A study analyzing these levels of 1150 pregnant people was done. Someone involved in this study further looked at the progression of the condition- did it get better or worse? It's thought of as getting worse if higher levels of TBA are found. Some of the 1, 188 in this study were divided in three bags based on the severity of their TBA levels. A whopping 354 of them (about 30%) had their TBA levels measured more than once. Twenty five percent of them experienced a surge in TBA levels. This could have resulted in deciding to deliver the baby earlier. The evidence strongly suggest that one-size-fits-all approach to gestation might not be the best.

The generation of more research may aid us in solving the problem of when to assess the TBA levels and reclassify the severity of the ICP. It is important to note that the severity of a condition is not always indicative of its likelihood to progress. The cause of the condition needs to be examined in order to better predict its path. More studies are needed in order to better understand how and when to assess the TBA levels to yield the best pregnancy outcomes. What is not yet clear is if frequent measurements of TBA levels and reclassification based on those measurements is necessary or even beneficial to the outcome for mom and her baby.
